This role focuses on comprehensive skin and wound assessment, treatment planning, and implementation of advanced wound care therapies. Qualifications: + Current RN license in good standing. + 18 months recent experience in wound care, including advanced therapies and technologies. + Strong assessment, documentation, and communication skills. + Ability to prioritize and complete tasks within assigned shifts. + PREFERRED Credential- Wound Care certification by VA approved accredited agency: + National Commission for Certifying Agencies (NCCA) or + Accreditation Board for Specialty Nursing Certification (ABSNC) Key Responsibilities: 1. Patient Assessment & Care Planning 2. Assess, plan, implement, and evaluate care based on skin and wound assessment findings. 3. Assess healthcare needs of patients/residents and develop care plans to meet interdisciplinary goals. 4. Identify and act on significant changes in patient/resident health conditions. 5. Update patient/resident care plans and notify physicians of any changes in condition. 6. Clinical Care & Documentation 7. Perform and delegate tasks according to the plan of care. 8. Collect and document pertinent data in the electronic medical record. 9. Complete nursing assessments within required time frames and in accordance with policy. 10. Provide timely and accurate documentation of skin and wound issues using VA-approved EHR systems. 11. Wound Care Expertise 12. Perform full scope of wound assessment and management, including measurement, staging of pressure injuries, cleansing, compression therapy, and application of wound care treatments. 13. Complete comprehensive skin and wound assessments and recommend appropriate prevention and/or treatment interventions (including ostomy and continence care). 14. Recommend treatments such as wound dressings, advanced therapies, specialty mattresses, and innovative technologies (e.g., mobile applications, 3D imaging, Telewound). 15. Patient Advocacy & Support 16. Meet the physical, emotional, and spiritual needs of patients/residents and families. 17. Report incidents of verbal or physical abuse immediately to the Nurse Manager or designated leadership. 18. Leadership & Delegation 19. Delegate elements of care based on assessed needs, patient condition, and safety. 20. Code Minimum Data Set assessments in Caribou and initiate/update care plans.
