Direct Care & Case Management:Provide direct and supportive health care to individuals in residential programs, assessing current health status and identifying needs.u2022 ? Health & Safety Oversight:Provide professional advice and direction on health and safety issues, ensuring the ongoing support of individualsu2019 health and medical needs.u2022 ? Collaborate with Treatment Team:Establish positive working relationships with individuals, families, and interdisciplinary team members, both within and outside the agency.u2022 u26a0ufe0f Emergency Response:Initiate nursing interventions in emergency and non-emergency situations, adhering to established policies and clinical standards of care.u2022 ? Documentation & Care Plans:Establish and maintain nursing care plans, document individual health needs, and follow up on required actions.u2022 ? Appointment Scheduling:Ensure timely scheduling of medical appointments and services for individuals in your caseload.u2022 ?u200du2695ufe0f Staff Training:Provide training to staff on nursing care plans and agency in-services, ensuring proper care delivery.u2022 ? Medication Management:Oversee medication administration, ensuring proper labeling, storage, and disposal of expired medications according to policy and regulations.u2022 ? Hospitalization & Discharge Coordination:Communicate with hospital discharge planners and ensure safe discharges for individuals who have been hospitalized.u2022 ? Health Record Monitoring:Track and update health records, including seizures, bowel movements, vital signs, and other health metrics.u2022 ? After-Hours On-Call:Be available for after-hour on-call duties, responding to situations in accordance with policies and regulations.u2022 ?ufe0f Safety Compliance:Follow safety guidelines to ensure a safe working environment and report any hazards.u2022 ? Program Coverage:May be required to float or cover other programs as needed or adjust work schedules to meet the needs of individuals. What Weu2019re Looking For:u2022 u2705 Education:Bacheloru2019s or Masteru2019s degree in Nursing, with at least one year of professional experience in delivering health care to individuals with Intellectual/Developmental Disabilities.u2022 u2705 Licensing:NYS RN Certification is mandatory.u2022 u2705 Driveru2019s License:Valid unrestricted NYS Driveru2019s License and reliable transportation for travel between locations.u2022 u2705 Skills:Ability to triage urgent and crisis situations, problem-solve, and maintain focus in challenging situations.u2022 u2705 Flexibility:Must be flexible with days, hours, and location based on agency needs.u2022 u2705 Vaccination:Must be fully vaccinated (COVID-19).
