This individual will report to the Transition Care Coordinator Clinical Supervisor. This individual will work with the Hospitalist team, clinicians, peers, community colleagues and others, to provide exemplary care across the health care continuum; to identify opportunities to continually improve patient care services, population health and, to achieve collaborative practices.
