Research Support Specialist ANNOUNCEMENT OF NON- PROFESSIONAL VACANCY Applications are invited for consideration to the following position Department: Medicine / STAR Program Budget Title: Research Support Specialist Salary Range: (Based Upon Experience)$50,000.00 - $51,500.00 Descriptive Title:Patient Navigator Full-time, Monday through Friday, 9:00am - 5:00pm, 37.5 hours per week. Vacancy 25-27 Description of Duties: This is a grant funded position. -Complete patient intake assessments, service plan development, reassessments and service plan updates as scheduled for patients and assigned by the program -Conduct out-reach and in-reach activities -Facilitate supportive individual (and group as needed) counseling sessions for a small caseload assigned by the program coordinator -Remind patients of medical appointments, conduct missed appointment follow-up to reengage and assist with linkage to care -Assist patients in completing forms for services including eligibility for health insurance -Update program PCSM's, record patient encounter notes in EMR and complete eSHARE data forms -Assist Behavioral Case Manager and other program staff with client assistance activities -Refer and accompany patients to medical and social service appt's at SUNY DMC and other community agencies -Participate in interdisciplinary case conference activities, outcome evaluation and quality improvement processes Qualifications: REQUIRED: A.A. or A.S. at least 2 years of behavioral health case management experience working in a community based agency or health care organization. Effective computer, verbal, organizational and interpersonal skills required. Must be computer literate with a proficiency in word processing. PREFERRED: B.A. or B.S. degree with a minimum 1 year counseling/health promotion experience in a community based agency or health care organization preferred. Bilingual Creole or Spanish speaking an asset. Field based and outreach experience.
