Job Summary TheRegistered Nurse at Prevea plays a crucial role as an integral member ofour clinic team, delivering patient-centered care. Responsible fortriaging incoming calls from patients and families, assessing theirneeds, giving appropriate advice, and facilitating appointments withphysicians and advanced provider clinicians. The Registered Nurseposition will also provide hands-on direct patient care as needed. TheRegistered Nurse is accountable for the delivery of exceptional and safecare within the scope of practice as defined by the Wisconsin Board ofNursing and Prevea's policy. Through collaborative practice withmembers of the Prevea team, they are responsible for patient outcomesthat meet the high quality of care provided by Prevea. What you will doManages patient/family health concerns through telephone triage andconsultation. Actively seeks information to understand patientcircumstances, problems, expectations, and needs. Facilitates caller'saccess to appropriate level of care by assessing needs and offeringclinical options using medial judgement within scope of licensure. Mayalso refer to health care providers, services, and community resources.Collaborate with other healthcare providers to develop and implementpatient care plans to ensure the best possible care and outcomes ofpatients. Conducts telephone calls back to patients requiring follow-upcare or provide test results. Also provides health counseling andeducation to patients and their families about their healthcare and anyfollow-up instructions. Provides direct patient care which includes, butit is not limited to the rooming of patients, obtaining medicalinformation, performing comprehensive initial patient assessment, andmaintaining patient charts/records. Assists the Physician or AdvancedPractice Provider with procedures, notes their instructions/ordres andmakes necessary arrangements to ensure they are carried out. Directpatient care may also include medication administration, andcommunicating information with patients, Physician, Advanced PracticeProviders and staff to ensure quality and continuity of care.Education...
