Job Summary At Prevea Health.the Access One Registered Nurse serves as a critical liaison betweenreferring and accepting facilities, ensuring efficient coordination andcommunication for patient consults and transfers into HSHS Wisconsinhospitals. This position requires advanced nursing judgment, strongtelephonic triage skills, and proficiency in multiple systems tofacilitate high-quality, timely patient care. The RN works closely withproviders, nursing supervisors, registration staff, and externalfacilities to ensure smooth patient transitions and adherence toorganizational protocols and is also responsible for handling initialtriage for Prevea departments outside of regular clinic hours. What youwill do Act as the central point of contact for incoming transfersand provider consults; obtain, enter, and verify accurate patientinformation for registration and tracking. Connect referring andaccepting providers using multi-line conference calls, paging systems,and online on-call schedules to ensure efficient patient hand-offs. Work closely with nursing supervisors, charge nurses, and registrationstaff to determine bed availability and ensure timely assignment ofaccount numbers. Accurately document all interactions and transferdetails in Epic, following workflows and protocols for various transfertypes. Request and receive medical records and diagnostic images fromreferring facilities to support care coordination. Assess patientsymptoms, provide care advice, initiate protocols, and triage patientsto appropriate levels of care using clinical guidelines. Processmedication refills per protocol, review EMRs, and relay critical labresults to on-call providers promptly. Provide primary triageservices for Prevea departments during non-clinic hours. EducationQualifications Graduation from an accredited and approved Nursingprogram Required Bachelor's Degree in Nursing Preferred ExperienceQualifications 1-3 years Registered Nurse experience Required Skillsand Abilities Use nursing assessment and decision-making skills topromote optimal patient outcomes and ensure timely, appropriate care.
