Psychoeducation Specialist u2013 St. Charles Hospital Job Summary: The Psychoeducation Specialist provides direct patient care under the supervision of a designated healthcare professional in accordance with federal, state, and local regulations, and within policies, procedures, and guidelines of Bon Secours Mercy Health (BSMH). Essential Functions: + Facilitates individual and/or support groups (within Psychoeducation scope) and may participate in family meetings and treatment team meetings. + Provides community resource knowledge and handles coordination of related activities. + May provide 1:1 patient safety companion service as needed, and patient checks as assigned. + Care planning and discharge planning in collaboration with Social Worker team. + Participates in continued education and in-service training.
