Navigator Homes of Marthas Vineyard - Position: Certified Nursing Assistant Employee: Supervisor: Nursing Supervisor & Director of Nursing Date of Hire: Department: Navigator Homes - CNAs PURPOSE OF YOUR JOB DESCRIPTION: The primary purpose of your position is to provide each of your assigned residents with routine daily nursing care and services in accordance with the resident’s assessment, care plan, and Windemere’s established nursing care procedures. DELEGATION OF AUTHORITY: No Delegation of Authority. JOB FUNCTIONS: Every effort has been made to make your job description as complete as possible. However, it in no way states or implies that these are the only duties you will be required to perform. The omission of specific statements of duties does not exclude them from the position if the work is similar, related or is a logical assignment to the position. PERFORMANCE REQUIREMENT / DUTIES AND RESPONSIBILITIES Requirements: 1= Exceeds all 2 = Meets All 3 = Meets Few The goal is for employees to have mostly "2"s. A "1" can be given only occasionally and for extraordinary performance. HIGH 1. 2. 3. LOW COMMENTS Record all entries on flow sheets, notes, charts, etc., in an informative and accurate manner. Report all accidents and incidents you observe during the shift in which they occur. Use only authorized abbreviations established by this facility when recording information. Report all changes in the resident's condition to the staff nurse as soon as possible. Ensure that the resident's room is ready for an admission, including bed made, necessary supplies in room, etc. Inventory and mark the residents personal possessions as instructed. Assist residents with packing their personal possessions when they are being transferred to a new room or when being discharged. Follow work assignments and/or work schedules in completing and performing your assigned tasks. Reports any complaints/concerns made by the resident to the staff nurse. Perform all assigned tasks in accordance with facility established policies and procedures, and as instructed by your supervisors. Notify the facility when you will be late or absent from work at least two hours prior to the start of shift. Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the unit and shift. Participates in walking rounds at the beginning and end of each work shift and receive/provide report. Takes temperature, pulse, respirations, & blood pressure and reports to nurse. Notes resident intake and output accurately and on appropriate forms. Changes and toilets incontinent residents as needed and keeps them clean and dry. Uses Universal Precautions at all times, in order to control the spread of infection. Bathes residents in special tub as indicated by the care plan. Weighs residents as directed and reports weights to nurse. Assist residents with daily dental and mouth care (brushing teeth/dentures, oral hygiene). Carries out cleaning assignments and extra duties as directed. Assists residents with dressing/undressing as necessary. Assist resident with hair care functions, including brushing, combing, and shampooing. Assist resident with nail care, including clipping, trimming, and cleaning. Shave male residents. Shave female residents as needed. Make beds (occupied and unoccupied). Position bedfast residents in correct and comfortable position. Check and report bowel movements and character of stools as instructed. Collect specimens as instructed (i.e., urine, sputum, and stool). Assist resident in preparing for medical tests (i.e. lab work, x-ray dental, etc.). Assist resident in preparing for activity and social programs and transport as directed. Assist with lifting, turning, moving, positioning, and transporting residents into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc. Answer residents call signals promptly. Assist with the care of the dying resident. Provide post-mortem care as instructed. Observe and report the presence of pressure areas and skin breakdown to prevent decubitus ulcers. Provide daily indwelling catheter care and perineal care. Provide daily range of motion exercises, record data as instructed. Turn all medications found in the residents' room over to the staff nurse. Prepare residents for meals (i.e. take to bathroom, wash hands, comb hair, raise bed, position tables, place clothing protectors, take to and from dining room, etc.). Serve food trays. Assist with feeding as indicated (i.e cutting foods feeding, assist residents with sight problems to identify food arrangements etc.). Record the resident's food intake. Notify nurse of any change in eating habit or of intake Serve residents fresh water. Serve between meal and bedtime snacks and record as indicated. Perform after meal care (i.e. remove trays, clean residents’ hands and face, remove clothing protector, brush teeth &/or dentures). Attend and participate in scheduled training, educational and orientation programs. Only use restraints as directed by nurse, releasing restraint every two hours and checking restrained resident every 30 minutes. Performs special skin care, decubitus prevention and diabetic foot. Assists in orientation of newly employed certified nursing assistants as directed. Participates, as assigned, in the external and internal disaster plan. Participates in ongoing QA program for the unit. Maintains compliance with OBRA regulations. Assists QA and QI Committee in developing and implementing appropriate plans to correct identified deficiencies. Maintains CONFIDENTIALITY of all pertinent resident care information to assure that resident rights are protected and maintaining HIPPA. Projects a positive attitude and respect toward staff and the facility. Has been educated in and adheres to standard precautions; demonstrates good hand washing procedures. Demonstrates the ability to work in a team and offers support to others. Dresses in accordance with the established dress code policy; maintaining professional appearance. Demonstrates ability to work without close supervision. Communicates effectively with coworkers, residents, families, and visitors. Ensure that all nursing care is provided in a respectful and dignified manner while maintaining privacy. Always knock before entering the resident's room. Use only equipment that you have been trained to use, and operate this equipment in a safe manner. Report defective equipment to staff nurse. Inform the staff nurse of any change in resident's condition. Reviews care plans to determine if changes in the resident's daily care routine have been made on the care plan. Keep floors dry. Report spills immediately. Keep resident's possessions off the floor and properly stored. Keep the residents call signal/light within easy reach at all times. Follow established safety precautions in the performance of all duties. EXPERIENCE: Geriatric and/or Long Term Care experience preferred but not required of serving the same age/type of resident population served by this facility. New graduate CNA’s will be considered on an individual basis. WORKING CONDITIONS: Work in office areas as well as throughout the facility. Walks, sits, stands, bends, lifts and moves intermittently during working hours Is subject to frequent interruptions Is involved with patients, residents, staff, government agencies/personnel, etc. under all conditions and circumstances Works beyond normal working hours, on weekends, and holidays when necessary. Is subject to hostile and emotionally upset patients, family members, etc. Communicates with the medical staff and other hospital departments’ staff Attends and participates in continuing training programs as they relate to the position Is subject to potential falls, burns from equipment, odors, etc., throughout the work day. May be exposed to infectious waste, diseases, conditions, etc., including the AIDS and Hepatitis B viruses. Is subject to lifting, carrying, and supporting patients Must be constantly alert for patient and resident safety Maintain a liaison with other department supervisors to adequately plan. SPECIFIC REQUIREMENTS: Must possess a current, unencumbered CNA certification to practice in Massachusetts. Must be able to read, write legibly, speak clearly and understand the English language Must possess the ability to make independent decisions when circumstances warrant such action in a timely fashion Must possess the ability to deal tactfully with staff, patients, family members, visitors, government agencies/personnel and the general public Must possess leadership ability and the willingness to work harmoniously with professional and non-professional personnel and the general public Must possess the ability to plan, organize, develop, implement, and interpret the program goals, objectives, policies, procedures, etc. of the QI/IC department. Must maintain the care and use of supplies, equipment, etc., and maintain the appearance of the department. Must have patience, tact, cheerful disposition and enthusiasm.
