Key Responsibilities + Monitor the health and well-being of program participants, ensuring the highest quality of care. + Administer medications, maintain accurate documentation, and oversee staff who assist with medication administration. + Provide prescribed medical treatments and personal care in accordance with physicianu2019s orders. + Conduct assessments, develop and implement health care plans, and respond appropriately to changes in condition. + Train staff on topics such as infection control, universal precautions, and medication administration. + Serve as a liaison and advocate between participants, families, physicians, and community service providers. + Educate participants on topics such as self-medication, smoking cessation, and STD prevention. Qualifications + Current Massachusetts LPN or LVN licensure required. + Strong assessment, communication, and clinical documentation skills. + Ability to work collaboratively with staff, participants, and families. + Prior experience in an adult day health, community health, or long-term care setting preferred.
