Assist Outpatient clinicians with intakes and referrals for treatment and recovery services Help clients complete intake paperwork and consent forms Provide a warm welcome to the clinic and support a positive, trauma-informed first contact Raise awareness of available social, recovery, and support services Model healthy coping skills and offer non-clinical crisis support, especially following hospitalization or incarceration Conduct deep outreach to individuals struggling to stay engaged in treatment, including in-community visits, contact with family members (when appropriate), and coordination with outside providers Assist with higher-level-of-care referral support Accompany clients to appointments when approved by Supervisor Work with clients to identify strengths and personal recovery goals Link participants to formal and informal recovery supports Help coordinate care among treatment providers, care managers, probation/parole, medical providers, and others Provide 1:1 peer support and engagement outreach Attend and support outpatient groups, offering education on recovery pathways and community resources Facilitate a peer support group for clinic participants Participate actively in Outpatient Multidisciplinary Team Meetings Maintain an organized and sanitary work environment Maintain active professional licensure/certification, completing renewals and required continuing education Occasional client transport to support recovery goals Other duties as assigned Work performed primarily in an office environment Daytime schedule, part-time (up to hours/week) Reliable transportation is required for occasional community-based outreach and transport Clean driver's license High School Diploma or GED (unless waived by OASAS) ASAP-NYCB Peer Advocate certification (CRPA or CRPA-P) Ability to use a computer/iPad for documentation Lived experience with substance use and recovery Reliable transportation and a clean driving record Equivalent combinations of education and experience may be considered Strong ability to engage clients with complex challenges, including serious mental health conditions and criminal justice involvement Strong written and verbal communication skills Familiarity with community-based resources and supports Ability to work independently while contributing to a collaborative team environment Demonstrates cultural humility and strengths-based practice
