New Horizons operated the only first two and longest standing Qualified Residential Treatment Programs (QRTPs) in the entire stateu2014demonstrating our commitment to high-quality, trauma-informed residential care for youth with complex needs. In addition to our QRTPs, we offer a fully accredited Child Placing Agency (CPA), supporting safe and nurturing foster and adoptive homes. Our Family Programs Division provides critical community-based services through: + FAYS (Family and Youth Success) u2013 strengthening families and helping at-risk youth + Kinship Services u2013 supporting relatives who step in to care for children in need + Family Resource Center (FRC) u2013 providing wraparound support and local access to resources for families in crisis or transition Together, these programs form a robust, integrated support system focused on building resilience, restoring hope, and achieving long-term positive outcomes. At New Horizons, you won't just be joining a teamu2014youu2019ll be joining a movement to protect and uplift children and families across Texas. The Aftercare Case Manager provides ongoing support and coordination for youth and families as they transition from residential treatment to community life. This role ensures continuity of services, connects families to needed resources, and helps youth continue their healing journey through structured aftercare planning. The Case Manager collaborates with the Service Planning team, assists with school enrollment, medical and mental health provider transitions, and tracks aftercare outcomes to promote long-term stability and success. This role is based out of Abilene. Key Responsibilities + Collaborate with the Service Planning team to create and monitor individualized aftercare plans. + Manage a caseload of approximately 25 youth receiving aftercare services. + Conduct and participate in aftercare check-ins to assess progress and needs. + Assist with school enrollment and maintain educational portfolios. + Coordinate medical and mental health service transitions to ensure no lapse in care. + Document all services and outcomes accurately and timely in accordance with agency and state standards. + Support parents and caregivers in accessing community resources and normative, age-appropriate activities. + Adhere to all New Horizons policies, state, and federal child care regulations. + Maintain a commitment to New Horizonsu2019 philosophy of Keeping Kids First in all actions and decisions. Qualifications + Bacheloru2019s degree and two years of experience working with youth who have experienced trauma preferred; or a masteru2019s degree in a social service or related field. + Valid driveru2019s license, proof of insurance, and ability to transport youth as needed. + Strong organizational, communication, and documentation skills. + Ability to build positive relationships and work collaboratively across disciplines. + Understanding cultural diversity and the ability to work effectively with diverse populations. + Must pass a criminal background check, drug screen, and tuberculosis screening.
