In this vital role, you’ll work closely with residents, families, and our interdisciplinary team to ensure each resident’s psychosocial, emotional, and mental health needs are met. You’ll play a key part in fostering a positive environment that promotes individuality, independence, and dignity — helping every resident achieve their highest level of well-being and quality of life. Key Responsibilities: Identify and address residents’ psychosocial, emotional, and mental health needs. Assist residents and families in accessing community resources and services. Complete admission assessments and develop individualized care plans. Lead discharge planning and ensure smooth transitions of care. Participate in quality assurance meetings and interdisciplinary team planning. Ensure compliance with all state and federal regulations. Maintain accurate and timely social service documentation. Promote and protect resident rights while preventing and addressing abuse or neglect. Develop and update facility policies to enhance resident care and quality of life. Qualifications: Bachelor’s Degree in Social Work or related field (LSW or LISW preferred). Experience in long-term care or healthcare setting strongly preferred. Excellent communication, organizational, and advocacy skills. A compassionate approach and commitment to resident-centered care.
