ACT Housing Specialist

This job was posted by : For moreinformation, please see: a member of an Assertive Community Treatment (ACT) team, functions asthe housing specialist with primary responsibility for the provision ofhousing support services in accordance with SAMHSAs evidence-basedpractice of permanent supportive housing. Provides comprehensiveassessment of housing, residential and activities of daily living skillsand needs, and leads in service planning for adult daily living skillsand housing support services. Coordinates, monitors, and provides directservices to members in a community setting. Participates in thedevelopment of individualized service plans (ISPs) andprovides/coordinates services related to ISPs. Provides after-hourscare, on-call coverage, and support to all members being served by theACT team.
