Description Care Coordinator - Children's LongTerm Support (CLTS) Waiver Program Milwaukee County, WI Full-Time Monday-Friday, First Shift About Us Lutheran Social Services ofWisconsin and Upper Michigan (LSS) is one of the largest nonprofit humanservices organizations in the Midwest. We are excited to launch a new Milwaukee County team and areseeking Care Coordinators to support children with developmental,physical, or severe emotional disabilities through the Children's LongTerm Support (CLTS) Waiver Program. Role Overview As a CLTS CareCoordinator, you will serve as the primary point of contact forfamilies, helping them access services that keep children safe andthriving in their home and community. This role involves assessment,service planning, coordination, and ongoing support. KeyResponsibilities Conduct functional assessments and reassessments usingapproved tools Develop individualized service plans with families andproviders Authorize and coordinate services in alignment with the planFacilitate family-centered team meetings Maintain accurate documentationand client records Collaborate with service teams and supervisors todetermine interventions Testify at legal proceedings when requiredParticipate in staff development, supervision, and training DirectService Expectations Promote client independence and growth Providetrauma-informed, family-centered services Utilize evidence-basedpractices supported by LSS and funders Complete time reportingaccurately and on schedule Qualifications Bachelor's degree in a healthor human services field (examples: Social Work, Psychology, Counseling,Special Education, Occupational Therapy, Sociology, Criminal Justice,etc.) Minimum of 1 year of...
