Offer Based on Years of Experience WhatYou Need to Know Be the Best Hospice Registered Nurse/RN Team DirectorYou Can Be If you meet these qualifications, we would love to meet you:Associate degree preferred Certified Hospice Nurse (CHPN) preferredThree+ years' experience in hospice, home health, general medicalsurgery, oncology, or patient population-specific nursing (i.e.,geriatrics, pediatric, etc.) Minimum of one year of supervisoryexperience Possess and maintain valid CPR certification while employedin a clinical role (preferred) Must be a licensed driver who can travelto all business locations Meet the regulations and requirements of thestate(s) in which the program provides services Responsibilities: As aHospice Registered Nurse Team Director, you will: Supervise directpatient and family care team members, in collaboration with program siteleadership Responsible for scheduling , payroll, and otheradministrative duties Ensure all initial assessments and requiredreassessments are conducted by qualified team members within requiredtime frames, including documentation submissions, and/or synchonizationOversee and support the Interdisciplinary Group (IDG) in development,implementation, and review of the interdicisiplinary plan of care foreach patient and family Collaborate with program site leadership memebrsto monitor, manage, and mentor staff in patient care activities,productivity, and documentation per related policies and protocolsPariticpates and promotes Back to Bedside efforts in reinforcing CaseManagement skills, and knowledge and attitude per protocol for alldisciplines Collaborate with site leadership to support staff educationefforts Facilitates IDG meetings and...
