Job Responsibilities: Work under general direction with physicians and other multidisciplinary team members to develop plans for the care for designated patients Ensure patients are progressing to-ward desired outcomes by monitoring care through assessments, patient and family interactions and re-view of patient records Identify and resolve barriers that hinder effective and timely patient care and discharge Advocate for the patients Be actively involved with our amazing Social Work team and others in the discharge planning process Hours/Schedules: Full-time, 8-hour shifts Monday-Friday Day shift Minimum Requirements: Licensed to work as a Registered Nurse in Wisconsin 3-5 years of hospital clinical experience as a registered nurse Proven ability to function effectively with others to develop and implement a plan to address the needs of the patient during hospitalization and after discharge About us: Froedtert South is a comprehensive regional healthcare system that has served southeastern Wisconsin and northern Illinois communities for more than 120 years.
