Adecco Healthcare & Life Sciences is assisting our client hire an RN Care Manager in Rochester, New York This opportunity is a full time, direct hire position. This role is hybrid. Location: Rochester, NY - Hybrid, mix of telehealth and home visits Type: Direct hire, full time offer with client Schedule: Monday through Friday, 8am to 5pm Pay: $73,500 to $77,500 yearly Responsibilities of the RN Care Manager: u00b7 Providing care organization, education, resource navigation, and clinical support for pediatric and OB/GYN patients to reduce health disparities, support population health goals and the overall improvement of clinical outcomes in patient populations most at risk. u00b7 This position includes the ability to provide care via telehealth and conduct home visits as appropriate. u00b7 Ensure patients receive consistent, competent, high-quality care u00b7 The organization achieves targeted health outcomes u00b7 Regulatory and financial goals are met u00b7 Access and engagement are optimized Qualifications for the RN Care Manager: u00b7 RN license in NY, active and in good standing u00b7 Two years of nursing experience, one year in pediatrics, OB/GYN or community/public health u00b7 Proficiency with EHR and data documentation u00b7 BLS through the American Heart Association u00b7 NYS Infection Control certification Preferred Qualifications: u00b7 Experience in care coordination in an FQHC or similar safety-net setting. u00b7 Bilingual in English and Spanish (or other prevalent language). u00b7 Certification in Care management (e.g., CCM, RN-BC), maternal-child or pediatric nursing. u00b7 Familiarity with HRSA metrics and Medicaid Manage Care.
