We are seeking acompassionate and motivated Care Coordinator to join our SHINE programteam. As a Care Coordinator, you'll work as part of a multidisciplinaryteam to help youth and families develop individualized care plans thatguide them toward their vision of being happy, healthy, and safe. You'll also serveas the link between families, providers, and the SHINE team to supportcomprehensive interventions and help youth achieve their goals.WhatYou'll Do Facilitate all aspects of services for enrolled youth,including transportation, needs assessment, team assembly, developmentof Plans of Care, crisis planning, and securing additional services whenneeded Coordinate care across mental health (CCS) and disability (CLTS)systems in collaboration with SHINE providers Monitor, update, andadjust care plans in response to changing needs, modifyingauthorizations as required Ensure compliance with all legal,organizational, and contractual requirements Complete and submit allrequired documentation by established deadlines Establish and maintaineffective communication and collaboration with families, providers, andstakeholders Serve as a mandated reporter and follow all dutiesaccordingly Attend meetings, professional development, and trainings asrequested Contribute to a positive and collaborative team environmentWhat we are looking for: Bachelor's degree in social work or relatedfield (psychology, sociology, counseling) required Certified SocialWorker certificate preferred, master's degree in related field highlypreferred One (1) year of experience providing mental health servicesrequired Two (2) years of experience serving youth and young adults withmental health and developmental disabilities preferred Bilingual inSpanish and English (oral and written) desirable Strong...
