Position: RN / LPNProgram: 6400 Adult Residential Group HomeLocation: Delaware County, PAEmployment Type: Part-time / Full-time (Flexible Scheduling)About UsUnited Family Inclusive Living Center Inc. is a licensed 6400 Adult Residential provider in Pennsylvania committed to delivering high-quality, person-centered services for adults with intellectual and developmental disabilities. We are seeking a compassionate and dependable RN or LPN to support medical care coordination and regulatory compliance within our residential homes.Position SummaryThe RN/LPN will provide routine and specialized clinical oversight to ensure the health, safety, and well-being of individuals living in our group home. This role includes medication oversight, training of staff, assessments, chart reviews, and communication with providers and families, while maintaining compliance with PA 55 Chapter 6400 and ODP standards.Key ResponsibilitiesConduct health assessments and ongoing clinical monitoring of individuals.Review Medication Administration Records (MARs) for accuracy and compliance.Train Direct Support Professionals (DSPs) on medication administration per PA regulations.Support individuals in understanding their medical needs, diagnoses, and municate with physicians, pharmacies, specialists, and hospitals as needed.Ensure compliance with all regulatory documentation requirements under 6400.Review incident reports and provide clinical recommendations.Participate in Individual Support Plan (ISP) meetings as needed.Maintain accurate nursing progress notes and documentation.Monitor chronic conditions and administer treatments as required within scope of practice.Provide on-call consultation for medical concerns when scheduled.QualificationsCurrent Pennsylvania RN or LPN license (active and in good standing).Experience working with individuals with intellectual/developmental disabilities (preferred).Knowledge of PA 55 6400 Regulations (training provided if needed).Strong communication and documentation skills.Valid driver's license and ability to travel between program locations.CPR/First Aid certification (or willingness to obtain).Schedule & CompensationFlexible schedule: day, evening, or weekend hours availableHourly rate based on experienceOpportunity for long-term professional growth within the agencyWhy Join Us?Supportive and mission-driven leadershipA collaborative team that values respect, integrity, and high-quality careMeaningful work that directly improves the lives of individuals we serve
