Clinic Care Coordinator - Population HealthOur busy family medicine clinic is hiring a Clinic Care Coordinator to support population health and quality improvement. This role is perfect for someone who enjoys data, analytics, and helping patients stay on track with their care.What You’ll Do:Review reports to identify patient care gapsSupport quality measures and population health programsCoordinate patient outreach and schedulingAssist with chronic disease registries and pre-visit planningWork closely with providers and clinical staffWhat We’re Looking For:Strong analytical and organizational skillsComfortable reviewing data and spotting trendsGreat communication and teamworkMedical office or care coordination experience preferredEMR experience a plusSchedule: Full-time/would consider Part Time, Monday-FridayAbout Our Clinic:Our clinic's mission is to provide peak quality care, with a focus on giving patients the best options for their health and wellness needs.
