Norris is seeking an organized and energeticindividual to join our Clinical team as a Group Care Case Manager at ourMukwonago campus This is an exciting position that offers a variety ofopportunities to make a difference in the lifes of our youth. This rolewill oversee cases for both residential and shelter care programs. Underthe supervision of the Director of Clinical Services (DCS), the GroupCare Case Manager (GCCM) is responsible for providing case managementservices to youth, and coordinates the admissions process for allprogram referrals to include residential and other programs assigned.Additionally, the GCCM assists program leaders, clinicians, and theNorris Academy employees to ensure a therapeutic environment ismaintained on the hall and in the school setting, personalized treatmentneeds are addressed, trauma informed practices are utilized, and barrierneeds of youth and families are attended to in order to promotesustainable results in the areas of safety, permanency, and well-being.The GCCM position requires the individual is able to transport youthand/or families served in a Norris-owned or personal vehicle whenneeded. The GCCM position requires being able to work a flexibleschedule that is subject to change to meet the program's evolving needsat any time, and includes some evening hours, weekends and/or holidays.One late night per week is required. Service Planning & TeamingFacilitates regular communication and collaboration among schoolemployees, clinicians, family members, case workers, the courts,probation, psychiatrist and other key team members for treatmentplanning and quality care (i.e., prepares for and attends med reviews,identifies need and organizes staffing meetings, attends team meetings,as requested, etc.). Knows, implements and provides input on eachyouth's treatment, individual, and guidance plans. Is the point ofcontact for the placing Norris regarding treatment planning, overallprogress of the youth and identifying extraordinary needs/resourcesbeyond standard contract requirements.
