What Youu2019ll Do: + Deliver high-quality physical therapy services to patients in their homes + Evaluate patient needs, develop individualized plans of care, and monitor progress + Collaborate with physicians, caregivers, and interdisciplinary team members + Adjust treatment plans to ensure optimal functional outcomes + Supervise and guide support staff to help patients achieve therapy goals + Maintain accurate, timely documentation and treatment records + Serve as a clinical resource and advocate for physical therapy within the community What Weu2019re Looking For: + Graduate of an APTA-approved Physical Therapy program + Active, unrestricted Physical Therapist license in the state of practice + Minimum 1 year of experience in a healthcare setting + Comfortable working independently in a field-based role + Ability and willingness to travel within the assigned coverage area (approximately 50%) + Reliable vehicle, valid driveru2019s license, and current auto insurance + Ability to lift up to 50u2013100 lbs and perform physical tasks associated with patient care _We value the unique skills of veterans and military spouses.
