Provideclinical recommendations to physicians and facility staff regardingmedication therapy changes and optimizations. Maintain up-to-datepatient/resident profiles, including medical history, lab results, andcurrent medications (prescription, OTC, and home remedies). Ensurelaboratory tests required for monitoring high-risk medications areappropriately ordered, reviewed, and communicated to the healthcareteam. Collaborate with physicians, nurses, and other healthcareprofessionals to provide clinical and technical support. Document allcommunications and clinical interventions in the patient's medicalrecord, including interactions with families and caregivers. Implementand enforce policies and procedures for the safe procurement, storage,administration, and disposal of medications and biologicals. Ensurecompliance with all state and federal regulatory requirements related topharmacy practice and medication management.
