JOB REQUIREMENTS: Case Management Intake & Assessment Conduct intakeassessments to determine eligibility, strengths, and service needsEvaluate psychosocial, emotional, medical, and behavioral needs ServicePlanning Develop, implement, and update individualized service plans(ISPs) with measurable goals focused on housing, employment, health, andwellness within grant requirements Set and report out measurable goalswith input from the resident and interdisciplinary team Coordination ofServices Link residents with internal and external services such asmental health care, substance use treatment, education, employmentassistance, and housing resources Schedule and coordinate appointments,transportation, and follow-ups Monitoring & Evaluation Maintain accurateand timely case notes, records, and reports in compliance with agencyand funding requirements (e.g., HMIS). Regularly review and updateservice plans based on resident progress Track goals and outcomes, anddocument updates in case files Monthly unit inspections per HUDguidelines Crises Intervention Respond to resident crises promptly andappropriately Implement de-escalation strategies and connect residentswith emergency services if necessary Advocacy Advocate for residentrights and access to appropriate resources Help residents navigatehealthcare, legal, and social service systems Facilitate or coordinatelife skills workshops or group sessions on topics such as budgeting, jobreadiness, or wellness Documentation & Reporting Maintain accurate andtimely case notes, progress reports, and other required documentationDocument rent payments, keep accurate ledgers, leases renewed in theproperty management software Ensure confidentiality and compliance withprivacy regulations Collaboration Participate in staff meetings, caseconferences, and interdisciplinary team discussions. Communicateeffectively with supervisor, clinical staff, family members,probation/parole officers, and community partners Resident SupportProvide emotional support, guidance, and mentoring to residents Educateresidents on life skills, coping mechanisms, and independent livingstrategies Policy Compliance Ensure all activities comply withorganizational policies and procedures, as well as applicable laws andregulations Assist with audits, inspections, and licensing reviews asneeded Other duties as assigned or requested Property ManagementProperty Oversight & Maintenance Manage the day-to-day operations ofassigned properties Coordinate routine and emergency maintenance andrepairs Conduct regular property inspections to ensure upkeep andcompliance with safety standards Tennant Relations Serve as the primarypoint of contact for tenants regarding inquiries and complaints Overseethe leasing process, including tenant screening, lease negotiation,renewals, and terminations Enforce lease agreements and property rulesfairly and consistently Financial Management Collect rent and otherproperty-related fees in a timely manner Enter rents and manage theledger Legal & Regulatory Compliance Ensure all properties comply withlocal, state, and federal regulations, including fair housing lawsHandle evictions, lease violations, and legal notices in accordance withapplicable laws Record Keeping & Reporting Maintain accurate andup-to-date property records, including leases, maintenance logs, tenantcommunications, and financial reports Prepare regular management reportsfor property owners or senior management Key Performance Indicators CaseManagement 100% of case notes completed by the end of each week 100% ofPlan of Care goals created/reviewed/updated on time per HUD guidelines90% of clients achieving measurable progress on at least two of thethree items in their Plan of Care 50% reduction in client crisisincidents Property Management Maintain a 95% occupancy rate 100% of rentcalculations completed accurately and on time 100% of rec rtificationscompleted on time 95% rent collection rate every month OTHEREXPERIENCE AND QUALIFICATIONS: Qualifications A bachelor's degree insocial work, social services, or psychology. A master's is preferredMust be a certified tax credit specialist Must have two years ofproperty management experience Must have two years of experience in casemanagement, life skills, as well as conducting assessment and counselingservices Experience working with Veterans is preferred Experiencedelivering services to SUD, COD, or the homeless population is preferredOther Skills & Abilities Ability to communicate clearly and concisely inboth oral and written formats High levels of professionalism withdifferent stakeholders Skilled in public relations and partneringpractices Intimate knowledge of the veteran recovery community andissues Must be flexible, maintain composure in difficult situations, andhave a sense of humor Ability to relate to and interact with individualsfrom diverse cultural backgrounds Maintain confidentiality of writtenand verbal information Working Conditions General office environment.The working environment is generally favorable. Lighting and temperatureare adequate, and no hazardous or unpleasant conditions exist. Work isperformed in an office environment with standard office equipmentavailable. Travel will be about 30% and mostly local. Time will be spentat client residences. Physical Requirements The work is generallysedentary and requires standing and walking for up to 20% of the time.There will occasionally be bending, lifting, and carrying up to 10-15pounds.
