Provide families with information, support and assistance; and work closely with the therapists and Service Coordination, to ensure the child's and family's needs are clearly identified and services found to meet those needs u00a0Reports to the Associate Director of Early Intervention.Key Responsibilities- Explore family concerns, resources, and priorities and support the achievement of children and family outcomes.u00a0- Discuss with family and other professionals the family's strengths, concerns, priorities and desired outcomesu00a0- Link family with appropriate services and resources as indicated on IFSPu00a0- u00a0Coordinate services with other agenciesu00a0- Provide crisis intervention, when neededu00a0- Maintain accurate records for fee for service system and complete all necessary paperwork as required.- Act as a referral source for inquiries regarding services for children with developmental delays and disabilities.- Providing mentoring and support for intern or practicum placements- Attend training as required.u00a0- Performs other related duties as assigned.
