Offer Based on Years of Experience What YouNeed to Know Be the Best Palliative Care Nurse Practitioner You Can BeIf you meet these qualifications, we would love to meet you: Graduatedfrom an approved school of professional nursing One year experience as aNurse Practitioner is preferred Licensed to practice as a NursePractitioner in the state of agency operation Active DEA license in thestate where care is being provided. Possess and maintain valid CPRcertification while employed in a clinical role, preferred Must be alicensed driver who can travel to all business locations Meet theregulations and requirements of the state(s) in which program providesservices Responsibilities: As a Palliative Care Nurse Practitioner, youwill: Develop and implement clinical plans of care for patients withchronic and terminal illnesses Perform patient assessment, history andphysical; recommendations on aggressive symptom-management Monitorpatient progress; update the Medical Director, Team Director and teammembers; consult regarding discharge plannning; and patient/familyteaching Maintain communication with the supervisory physician (s) andattending physician to review patient records and care plans on aroutine basis Initiate communication with attending physicians, otherPalliative Care staff members and other agencies as needed to coordinateoptimal care and use of resources for patient/family May provide supportat time of death Prescribe medication as permitted by the state and/orthe State Board of Nursing Participate in continum of care for patientsthat are hospice appropriate and ongoing education programs.
