Some highlights of this opportunity include: u00b7 Participate in the decision-making process in relation to admission to hospice at home or in the inpatient setting u00b7 Participating as a member of the Interdisciplinary Team in the development of individualized care plans u00b7 Certify (re-certify when necessary) patients eligible for hospice care under the Medicare Hospice Benefit u00b7 Consult with attending physicians regarding pain and symptom management u00b7 Coordinate efforts with the attending physician to provide continuity of care u00b7 Develop and monitor the medical component of the hospice program u00b7 Participate in the development, coordination and implementation of hospice services and policies u00b7 Participating in the development of staff and volunteer training u00b7 Conduct home visits (in person and virtual) as a consulting physician when appropriate u00b7 Act as a hospice liaison and advocate with the national, state and local physicians, healthcare professionals, partners and community u00b7 Perform related administrative functions as required and/or assigned u00b7 Schedule: Forty-eight (48) on call weeks per year: Monday 7am through Friday 7pm. Including mandatory in person attendance of 1 Inter Disciplinary Team Meeting (IDT) per week.
