Essential Responsibilities Assessment & Care Planning: Identify carecoordination needs, risk factors, and service gaps; develop andimplement effective care plans; report abuse/neglect as tensive Care Coordination: Support members in higher levels ofbehavioral health care, facilitate transitions, maintain caseloads, andcoordinate interdisciplinary teams. Transition Assistance: Assist withdischarge/transition from acute or residential care; prioritize memberson Ready to Transition (RTT) list; coordinate care outside service areawhen needed. Compliance & Quality: Maintain CCO compliance,documentation, and program tracking; support quality assurance and bestpractices. Organizational Duties: Uphold mission, values, and policies;promote equity, diversity, and inclusion. RequirementsEducation/Experience: Master's in Social Work, Counseling, orBehavioral Health; 2+ years mental health or substance use experience;valid driver's license. Preferred: LCSW/LPC, CCM or CADC II/III;experience in OHP/CMS benefits, motivational interviewing, and carecoordination.
