Clinical Intake Evaluator - LPC or LCSW Boulder Community Health, Boulder, CO 80303 $33.82 - $50.73 an hour - Full-Time, 36 hours per week +$8.00/hr for night shifts +$2.50/hr for evening and weekend shifts. Boulder Community Health is seeking a Clinical Intake Evaluator who is responsible for assessment of patients in consultation and cooperation with team members. Responsibilities u2022Provides program information to client, providers and units u2014 either written or over the phone. u2022Triages calls to determine need for treatment assessment or referral based on clinical information. u2022Demonstrates understanding of financial information, third party payors and its effect on triage. u2022Provides consultation/problem solving to members of the Department of Psychiatry, medical staff, medical units of BCH and clinicians in the community. u2022Assists Boulder Community Hospital staff and community affiliates in the admission of their patients to BHS programs. u2022Facilitates assignments of a physician or clinician as appropriate for admission providing clinical and financial information. u2022Provides ongoing communication with Mental Health Partners to coordinate patient care. u2022Maintain ongoing communication with inpatient unit to coordinate patient care. u2022Review referrals from outside facilities for possible transfer to BCH Inpatient.BHS Services.
