Job Summary The RegisteredNurse at Prevea plays a crucial role as an integral member of our clinicteam, delivering patient-centered care. Responsible for triagingincoming calls from patients and families, assessing their needs, givingappropriate advice, and facilitating appointments with physicians andadvanced provider clinicians. The Registered Nurse position will alsoprovide hands-on direct patient care as needed. The Registered Nurse isaccountable for the delivery of exceptional and safe care within thescope of practice as defined by the Wisconsin Board of Nursing andPrevea's policy. Through collaborative practice with members of thePrevea team, they are responsible for patient outcomes that meet thehigh quality of care provided by Prevea. What you will do Managespatient/family health concerns through telephone triage andconsultation. Actively seeks information to understand patientcircumstances, problems, expectations, and needs. Facilitates caller'saccess to appropriate level of care by assessing needs and offeringclinical options using medial judgement within scope of licensure. Mayalso refer to health care providers, services, and community resources.Collaborate with other healthcare providers to develop and implementpatient care plans to ensure the best possible care and outcomes ofpatients. Conducts telephone calls back to patients requiring follow-upcare or provide test results. Also provides health counseling andeducation to patients and their families about their healthcare and anyfollow-up instructions. Provides direct patient care which includes, butit is not limited to the rooming of patients, obtaining medicalinformation, performing comprehensive initial patient assessment, andmaintaining patient charts/records. Assists the Physician or AdvancedPractice Provider with procedures, notes their instructions/ordres andmakes necessary arrangements to ensure they are carried out. Directpatient care may also include medication administration, andcommunicating information with patients, Physician, Advanced PracticeProviders and staff to ensure quality and continuity of care.Education...
