Together, we are making a difference, one life at a time.Your Mission in ActionThe Medical Coordinator arranges and coordinates appropriate health services, including urgent, emergent, and preventative health services, for children in care. This includes maintaining complete, confidential medical records for children, ensuring appropriate communication of health needs to case managers and clinicians, scheduling health care appointments, coordinating transportation to health care appointments, ensuring qualified interpretation and translation services are in place, and serving as a liaison for community health care providers. This role adheres to agency policies, licensing, and regulatory standards.Provide UC with routine health screenings and perform basic health assessment; assess each UC’s unique medical needs and ensure they are properly met in a timely manner; report routine medical needs to the medical, clinical, case management and direct care staff per Agency policy and licensing standardsAdminister medication and treatment to UC as prescribed or as needed in accordance with licensing standards and nursing practices; coordinate medical appointments and follow through on medications and treatment as prescribed; coordinate and oversee the prescribed nutritional needs and diet and fluid intake for UC as prescribedFacilitate trainings for staff, such as dispensing prescribed, over the counter and psychotropic medication, blood borne pathogens, and CPR/First AidCreate, update, and maintain accurate and up-to-date UC case files and records with information pertaining to nursing care, child’s medical history, treatment plans, and observations; chart all pertinent information in the UC case files/records in accordance with Agency policy and licensing standardsComplete monthly, quarterly, and annual reports as requested and to remain in compliance with licensing and funding standardsAssist with the assessment, development, and evaluation of a UC individualized service plan based on their needs and medical condition; use care plans as a basis for providing therapeutic care to UCRespond to all crisis situations in a timely manner; ensure all Program departments are properly notified in a timely manner; report incidents according to licensing standardsAssist with the safe and physical evacuation of UC during an emergency or natural disasterRole includes on-call responsibilities as schedule and as medical emergencies/needs may arise; consultation can be provided remotely or on-site based on needWork as a team player and exhibit professional behavior and a positive attitude with both internal and external clients that reflects positively on the Agency and is consistent with Agency policies and practicesMinimum QualificationsCurrent LPN/LVN license in Texas or higher health-related qualification (e.g. licensed resisted nurse (RN) with bachelor’s degree) OR Bachelor’s degree in relevant fieldOne (1) year of employment in public health or a healthcare setting with demonstrated experience in care coordination, health administration, or direct clinical careHigh School diploma or equivalentBilingual English and Spanish, (speak, read and write)Must be 21 years of age or older per licensing requirementsAbility to maintain a professional approach and build a rapport and trust with residents and their familiesConsistently follows established standards, policies, and procedures in providing nursing careStrong writing skills with the ability to document and maintain accurate UC files/recordsStrong customer service, interpersonal, and people skills; ability to work and interact with diverse groups of people and handle stressful and emotional situationsStrong relationship management skills with the ability to work with a team in a collaborative mannerStrong time management and organizational skills with the ability to pay very close attention to details and meet deadlinesAbility to resolve conflict and respond to resident issues with professionalism, tact, and empathyProficient in use of personal computers, Microsoft Word, Excel, and Outlook, and information systems used to document and record UC informationPreferred QualificationsOne (1) year of experience with pediatrics and/or adolescentsAssociate degree in a medical or science fieldTravelThis position may require travel up to 20%Perks at UpbringCompetitive PTO & paid holidays Health, dental, vision insurance & more 403(b) PlanEmployee Assistance ProgramDiscounted Gym Memberships Physical Demands & Work ConditionsThis position requires sitting and looking and using a computer for long periods of timePosition works in a child facility and has constant exposure to childrenPosition requires frequent standing, walking and time in units/cottages which are area where children resideLift, push, pull, move up to 50 lbsMay be required to apply safety intervention with youth who could weigh over 100lbs.The physical requirements for this position, with or without reasonable accommodation, are representative of those that must be met by an employee to perform the essential functions of this position successfully.
