FTE: 1.000000 Standard Hours:40.00 Shift: Shift 1 ShiftDetails: Typical hours 8-4:30 Job Summary: Serves as a resource for theFroedtert &MCW system hospitals and healthcare team in evaluatingpatients' medical needs and eligibility for an Inpatient Rehabilitation(IPR) level of care. Engages in care coordination rounds with the acutecare teams to expedite consults for IPR admissions and/or other PM&Rneeds. Reviews medical records, conducts pre-admission screens, andcommunicates timely with PM&R consult physicians, IPR MedicalDirector/admitting physicians and Admissions Coordinator to verifypatient eligibility for IPR and to coordinate safe and timely transfersto the inpatient rehabilitation unit. Responsible for developing andnurturing professional relationships with case managers, physicians andclinicians on acute care hospitals units. Educates staff, patients andfamilies on the goals of medical rehabilitation and fosters strongrelationships to build business through an increase of referrals andadmissions. Tracks referral activity, admissions decisions and efficiency metrics and reports results to the Rehabilitation Leadershipteam. EXPERIENCE DESCRIPTION: A minimum of 3 years of clinicalexperience in nursing or rehabilitation therapy is required. Clinicalexperience in Inpatient Rehabilitation and familiarity with hospitaladmissions procedures preferred. Previous marketing/sales experiencepreferred. EDUCATION DESCRIPTION: Bachelor's Degree in Nursing or arelated field required. SPECIAL SKILLS DESCRIPTION: Strong clinicalassessment skills, critical thinking skills and judgment. Highlyorganized, with the ability to manage time and prioritize work flow.Ability to be self-directed and work autonomously. Strong customerservice and interpersonal skills necessary to interact with allstakeholders, physicians, insurance representatives and referrers.Effective written and verbal skills. Computer skills including EMRnavigation, data management and presentations, and report writingpreferred. LICENSURE DESCRIPTION: Current state of Wisconsin licensureas a Registered Nurse, Physical Therapist, Occupational Therapist, orSpeech Language Pathologist required. American Heart Association (AHA)Basic Life Support for the Healthcare Provider (BLS) or an AHA approvedequivalent is preferred.
