Specifically, provide resources on landlord vacancies and work directly with clients to mitigate evictions, poor credit and criminal histories.Connect clients with vocational services, SSI/SSDI and public benefits, where applicable.Maintain legible and complete client files according to funder requirements. Ensure client data in HMIS is current, accurate and entered in real plete monthly reviews of data relevant to the project and provide updates to direct supervisor.Continually seek the improvement in the quality of the HOC operations and case management program.Maintain appropriate professional boundaries with clients and co-workers. Ability to adhere to an appropriate self-care plan.Maintain training to ensure up to date qualifications in best practices such as progressive engagement, motivational interviewing, mental health first aid, harm reduction and others as directed by supervisor.Uses appropriate administrative, fiscal, physical, and technical safeguards to ensure the confidentiality, integrity, and security of CCEW client protected health information (PHI), per regulations outlined in the Health Insurance Portability and Accountability Act of 1996 (HIPAA).Will regularly utilize all forms of communication (email, text, phone, etc.) and database practices necessary for this position and as directed by supervisor.Adhere to the te
