When you thrive, so does the community of care we're building together.Offer Based on Years of ExperienceWhat You Need to KnowBe the Best Hospice Nurse Practitioner You Can BeIf you meet these qualifications, we would love to meet you:Graduated from an approved school of professional nursing - Masters degreeCurrent and unecumbered advanced practice nurse license in the state(s) practicingOne year of experience as an NPCertified by a nationally recognized advanced nurse practice organizationPossess and maintain valid CPR certification while employed in a clinical role (preferred)Must be a licensed driver who can travel to all business locationsMeet the regulations and requirements of the state(s) in which the program provides servicesResponsibilities:As a Hospice Nurse Practitioner, you will:Initiate communication with attending physicians, other hospice staff members and other agencies as needed to coordinate optimal care and use of resources for the patient/familyMonitor patient progress; update the Team Director and team members; consult regarding discharge planning and patient/family teachingMaintain continuity of care in coordinating with the care team and case manager for patients with ongoing needsPrescribe medication as permitted by the state and/or the state Board of NursingPre-HospiceFace-to-face clinical and needs assesment at intake, preferably in the patient's home setting, inclusive of patient's medical, emotional, spiritual, social, and practical needsFace-to-face reasssessment done when needed and at least every 60 days. Plan of care development based on these assessments and incorporting the patient's and family's prioritiesInitiate communication with attending physcians, other hospice staff members and other agencies as needed to coordinate optimal care and use of resources for the patient/familyClinical supervision of the care plan including all medicine ordersCollaborate with the rest of the pre-hospice team including alerting the team to changes and newly identified patient needs as they ariseOur Investment in YouCaring for others starts with caring for you.
