PCA/HHA is responsible for assisting Hearts of Mine Community Health Incu00a0Non-Medical Home Care clients with their personal care needs, activities of daily living, home environmental support, and safety for the client includingu00a0oversight and supervision for all waivered clients;Responsibilities:Quality of Care:u2022 Provide clients with assistance with personal care activities including bathing, mouth care, denture care, skin care, grooming, bathroom care, transferring, ambulation, and dressing and undressing activities.u2022 Assist clients with housekeeping tasks including planning and preparing meals, feeding, light housekeeping, changing linens, making beds, and laundering clothes, and bedding.u2022 Assist client in safety needs using equipment (side rails, foot stools, wheelchairs, etc.).u2022 Follow the Plan of Care as indicated on the Aide Activity Sheet.u2022 Observe the client and report any changes or questions regarding his/her condition and report any changes or questions regarding care to the CEO or assigned Field Supervisor.Documentation:u2022 Complete all agency records and reports according to agency policy and procedure.u2022 Accurately document services provided in accordance with agency policy on the day services are rendered.u2022 Submit documentation in a legible and timely manner per agency munication:u2022 Establish and maintain effective communication regarding client's care with client, family, and supervisor.u2022 Communicates and works cooperatively with other members of health care team, including participating in case conferences as needed.Professional Development:u2022 Attends and participates in twelve (12) hours of in-service training annually, staff meetings, Quality Improvement activities and other meetings as requested; that includes HCBS Waiver-specific training.u2022Successful completion of a Pennsylvania State Health Department approved Personal Care Aide training program or successful completion of competency testing.
