Peer Support Specialist, Wellness West

Description Thresholds is seeking a compassionate and dedicated Peer Support Specialist to join our Wellness West Care Coordination Program. In this role, you will leverage your lived experience to support individuals navigating mental health and physical health challenges in person on the West Side of Chicago. Wellness West services individuals who suffer disproportionately from higher rates of chronic disease, acute ailments and mental illness. You will serve as a member advocate, helping individuals regain control over their lives and recovery processes while modeling resilience and coping skills.
