Nurse Practitioner Job Category: Healthcare Provider Requisition Number:NURSE001030 Full-Time On-site Springfield, MA 01103, USA Reports to:Chief Medical Officer Salary/Status: Exempt Minimum Requirements:Current Massachusetts NP licensure and /or eligibility forcertification. Current Mass CSR and DEA licenses Certification in FamilyPractice, Adult, or Pediatric. Ability to collaborate withmulti-disciplinary team, including physicians, nursing staff, socialworkers, and case managers. Previous experience in ambulatory careand/or public health is preferred. Demonstrate proficiency in assessmentand delivery of family planning services to a diverse population.Desirable: Fluency in Spanish, Arabic, Russian, Nepali, Vietnamese oranother language served at CHC Special expertise in provision of healthcare to underserved, culturally diverse populations PrincipleResponsibilities and Duties: Conduct comprehensive patient assessments.Identifies potential and actual health and health-related problemsEvaluates patient responses associated with the problem area(s).Diagnoses acute and chronic illness. Develops and implements treatmentplans that include pharmacologic and non-pharmacologic cludes the patient and family as active participants in treatmentplan. Focuses the NP/Patient interaction on wellness, health promotion,disease prevention, and self-care. Offers comprehensive clinical familyplanning services when appropriate. Reviews patient charts and ensuresappropriate treatment plans are developed and followed. Completes allcharts the same day of care or no later than 72 hours after the visit.Patient charts are completed in a manner that meets all risk managementand chart audit requirements. Collaborates and consults with CHCphysicians and other providers. When deemed necessary refers patients toappropriate specialists and other practitioners and facilities.Cooperates with efforts of CHC administration to monitor and improve thequality of patient care by evaluating patient outcomes. Maintainsupdated Nurse Practitioner guidelines. Participates in peer review ofcharts. Maintains all CME/CEU, Board and Licensing requirements of stateand national governing Boards. This position requires compliance withCaring Health Center's compliance standards including its Standard ofConduct, Compliance program, and policies and procedures. Suchcompliance will be an element considered as part of the NursePractitioner's regular performance evaluation. This position alsorequires the ability to use a computer, work at a computer workstation,viewing a CRT and have some knowledge of a keyboard.
