Job Description

The Unit Manager oversees the daily clinical operations of an assigned unit within the Skilled Nursing and Rehabilitation facility. This role ensures high quality resident care, regulatory compliance, staff performance, and effective coordination between nursing, therapy and interdisciplinary teams. The Unit Manager provides a positive work culture, leads quality improvement initiatives and supports the facility's mission of delivering excellence in post-acute and long-term care. What will I do every day?Oversee and supervise all nursing care delivered on the unit.Conduct daily clinical rounds to assess resident's conditions and identify needs for interventions or changes in care plans.Collaborate with physicians, therapy, social services and other IDT members to ensure timely and appropriate care.Coordinate admissions, discharges and transitions to promote optimal resident outcomes.Ensure accurate, timely documentation in accordance with federal, state and facility standards.Ensure compliance with CMS, state regulations and facility policies.Monitor and ensure adherence to infection control practices.Participate in care plan meetings, QAPI programs, audits and surveys.Identify and investigate resident incidents, develop correction actions and track outcomes.Monitor supply usage and ensure availability of equipment and materials.Manage, guide and evaluate nursing staff.Lead team meetings and promote communication, team work and accountability.Support the Director of Nursing and Assistant Director of Nursing with reports, documentation and special projects.As well as perform other related duties as required.What will I need to be successful in this role?Minimum 2-3 years of nursing experience in a skilled nursing, long-term care or post-acute rehab setting.Minimum 3 years of supervisory experience.Strong understanding of MDS, care planning and nursing documentation standards.Excellent clinical assessment and critical thinking skills.Ability to lead, motivate and develop staff.Strong communication, organizational and time management abilities.Familar with electronic medical records (ie., Point Click Care or similar).Special Requirements/Certifications I may need?Current active, unencumbered RN license in the state of practiceGraduate of an accredited RN/LVN/LPN program What’s in it for me?
