Location: 505 OAKFIELD DR, Brandon, 33511 The community youu2019ll be caring for: AdventHealth Medical Group Oakfield The value youu2019ll bring to the team: PRINCIPAL DUTIES AND JOB RESPONSIBILITIES : u00b7 Incorporates patient, physician, customer needs and concerns into decision-making and organizational action. u00b7 Performs initial and on-going assessments of self-care, psychosocial needs, education, environment and discharge planning, incorporating information from the identified significant other and other health providers. u00b7 Utilizes skills of observation, inspection, palpation, percussion and auscultation using diagnostic instruments or laboratory procedures that are basic to the screening of physical signs and symptoms. u00b7 Utilizes fetal monitoring screening for auscultating, monitoring, and assessment of fetal heart tones. u00b7 Assesses learning needs, readiness, and capabilities of patient/family, including cultural, religious practices, language barriers, emotional barriers, educational level, motivation and limitations. u00b7 Documents patient care findings within established standards and accreditation requirements . u00b7 Seeks appropriate resources to clarify information concerning the nursing/medical care regime as needed. Coordinates care by collaborating with physicians and others to obtain appropriate services, including multidisciplinary team meetings. u00b7 Exercises critical thinking skills to make sound clinical judgments about patient care as evidenced by practice patterns, documentation and patient outcomes. u00b7 KNOWLEDGE AND SKILLS REQUIRED : u00b7 Reassesses patients regularly and revises interventions/plan of care based on evaluation of effectiveness, change in medical condition and/or patient satisfaction. Documents the patientu2019s progress toward identified outcomes. u00b7 Reports changes in patient condition in a timely and effective manner. u00b7 Documents the patientu2019s condition and capability to manage continuing care needs in the discharge/transfer note. u00b7 Counsel individuals, families and groups about health and illness and promote health maintenance. u00b7 Supports and adheres to organizational policies, procedures and protocols. u00b7 Practice reflects knowledge and honoring of patient rights, Advance Directives and Ethics Committee referral process. u00b7 Demonstrates patient advocacy and the ability to promote patient satisfaction. u00b7 Honors patient dignity, privacy and confidentiality, and respect for human diversity. u00b7 Demonstrates collaborative behaviors with members of the health care team. u00b7 Promotes a positive image within hospital system and community. u00b7 Models behaviors conducive to the developmental needs of staff and helps them develop their potential.
