Facility: BayCare Health System, Rehab Services Main u2013 SJHN Location: St. Josephu2019s North - Lutz, FL Status: Pool (Non-Benefit Eligible) Exempt: No Shift Hours: Sundays 8:30 AM u2013 5:00 PM; weekdays as needed Shift: Days Weekend Work: Every Other On Call: No Physical Therapist (Pool) Responsibilities: + Perform comprehensive physical therapy evaluations and develop individualized treatment plans. + Deliver evidence-based interventions to improve mobility, strength, balance, and functional independence. + Collaborate with physicians, nurses, case managers, and other therapists to coordinate care. + Educate patients and families on exercises, safety, and post-discharge recommendations. + Document all patient care activities accurately and timely in the electronic medical record. + Participate in team meetings and contribute to quality improvement initiatives.
