Organization: VPH Social Work - PHP Job Summary: JOB SUMMARY Performs behavioral health diagnostic and psycho-social assessments utilizing the DSM-V, implements group, family and individual psycho therapeutic interventions including crisis intervention, develops treatment plans and disposition plans. Maintains contact with family and community agencies and makes referrals. Actively collaborates with team to meet patient care management and system improvement goals. Represents the behavioral health enterprise on institutional and community committees/task forces. . KEY RESPONSIBILITIES u2022 Provides initial assessment and develops plan of care according to TN Department of Mental Health, The Joint Commission and CMS Best Practice Guidelines and within the scope of their license by the Tennessee Health Related Boards. u2022 Provides targeted evidence based interventions into clinical practice with patients. u2022 Facilitates transitions to appropriate levels of care via in person, telephone or telehealth contact. u2022 Acts as clinical liaison with patient, family, treatment team and community. u2022 Contributes to the organization by actively participating in process improvement initiatives and shared decision making. u2022 Engages in regularly scheduled supervision as required by the Tennessee Health Related Boards. u2022 Upholds ethical standards in accordance with relevant professional associationu2019s code of ethics u2022 Maintains confidentiality and privilege in accordance with TN state codes and statutes TECHNICAL CAPABILITIES u2022 Patient Documentation (Intermediate): Demonstrates mastery of patient documentation in practical applications of a difficult nature. Possesses sufficient knowledge, training, and experience to be capable of successfully delivering patient documentation services without requiring support and instruction from others. Able to train and educate by setting the example, giving technical instruction, providing leadership, and generally raising the level of performance of others while on the job. u2022 Patient Diagnosis (Intermediate): Demonstrates mastery of patient diagnosis in practical applications of a difficult nature. Possesses sufficient knowledge, training, and experience to be capable of successfully delivering results without requiring support and instruction from others. Able to train and educate by setting the example, giving instruction, providing leadership, and generally raising the level of performance of others while on the job. u2022 Patient Assessment (Intermediate): Demonstrates mastery of patient assessments in practical applications of a difficult nature. Conducts prevention, contingency planning and preparation of alternatives for normal problems.
