Our Company Hospice of the Midwest Overview Office Location: 6600 France Avenue South, Ste. 210, Edina, MN 55435 Coverage area:Edina Schedule: Monday-Friday, 8:00am-5:00pm HOSPICE OF THE MIDWEST is seeking a passionate, dedicated Social Worker to join our team inEdina, MN. Our Social Workers provide emotional support, counseling, and resource coordination to patients facing terminal illness. This role plays a key part in the delivery of holistic care and ensures that patients' psychosocial needs are met with dignity and respect. Meets personnel qualifications stated in 418.114(b)(3). Minimum of one (1) years' experience in health care, hospice experience preferred. Understands hospice philosophy, and issues of death/dying. Experience in a hospice care preferred. Demonstrates good verbal and written communication, and organization skills. Current licensure according to Medicare guidelines to practice in state of operation. Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
