The Population Health Division (PHD) provides core public health services for the City and County of San Francisco: health protection, health promotion, disease and injury prevention, and disaster preparedness and response. Note: If you previously applied to a SFDPH Nurse Practitioner position before 9/26/25, you need to apply again to be added to this new eligible list. u00a0 Under general nursing and medical direction, utilizing standardized procedures, the Nurse Practitioner functions within an organized system for the delivery of health care in association with other members of the multidisciplinary health care team and performs specialized nurse practitioner duties in clinical settings. The essential functions include, but are not limited to: + Provides routine preventative and continuity of care to adult and/or pediatric patients; this may include complete history and physical examinations. + Orders diagnostic tests, which may include laboratory tests, X-rays and other imaging, E.K.G.u2019s, and other disease or condition specific diagnostic tests. + Identifies and manages patients with unstable chronic diseases appropriate to clinical specialty. + Provides urgent care to acutely ill patients. + Acute care planning for patients in an inpatient setting. + Assesses and diagnosis, develops treatment plans, and furnishes medications. + Counsels and educates patients, and collaborates with healthcare team. + Consults with physicians on the diagnosis and treatment of unstable and complex patient conditions. + Documents medical recordsu00a0and adheres to patient privacy. + Participates in staff meetings and conferences regarding patient care, as well as quality improvement/control activities.
