The Admission Nurse also develops relationships with facility and community referral sources and promotes exemplary customer service. Care delivery primarily takes place in the patient's home or in a facility where the patient receives care. Responsibilities u00a7 Facilitates smooth transition and admission of patients from health care settings into HOV programs. u00a7 Promotes HOV services and programs. u00a7 Develops and maintains therapeutic relationships. u00a7 Conducts comprehensive nursing assessment. u00a7 Plans care and intervenes effectively. u00a7 Promotes patient safety. u00a7 Provides effective patient/family/caregiver teaching. u00a7 Supports the death/dying process. u00a7 Manages end-of-life ethical issues effectively. u00a7 Collaborates in providing patient care. u00a7 Creates timely and accurate documentation. u00a7 Utilizes resources effectively and efficiently. u00a7 Adheres to scope of practice/HOV standards and facilitates continuously improved processes/services. u00a7 Maintains and enhances professional skills. u00a7 Adheres to high standards of personal and professional conduct. Minimum Qualifications u00a7 Graduate of accredited school of nursing. u00a7 Current Arizona RN licensure in good standing. u00a7 Minimum one year of recent experience in medical, surgical or critical care as a professional nurse. u00a7 Strong clinical assessment skills. u00a7 Excellent communication and interpersonal skills. u00a7 Solution driven, creative and resourceful problem solving skills. u00a7 Ability to manage time well and perform assigned duties with attention detail, accuracy and follow-through. u00a7 Proficient computer skills. Preferred Qualifications u00a7 Previous hospice or oncology experience. u00a7 Knowledge of the death/dying and bereavement process. u00a7 CHPN certification.
