We provide vital support during life's mostchallenging moments, helping individuals heal, grow, and thrive. We'reHiring We are seeking a Care Coordinator to join our team. Thisfull-time position is responsible for facilitating groups, providingcase management assistance, referrals, job search/promotion, clienttransportation, urinalysis administration, advocacy, providing childcareand Family and Parenting Programming, assisting with other programadministrative tasks, and assisting other Women's Recovery Journey(WRJ) team members while operating under WRJ Core Values. This positionwill split time between Brown County and Oconto County. KeyResponsibilities Assess participants' needs, resources, and supports byutilizing assessments Facilitate participant access to communityresources including (but not limited to): making referrals, providingreferral program and contact information, creating correspondence toclients and collaterals, scheduling appointments, assuringfollow-through with meetings and appointments, providing transportation,accompanying clients and their families to appointments to fosterwarm-hand offs. Advocate for participant and family to efficientlyobtain needed services. Collaborate with community and agency resourcesand maintain positive working relationships. Data entry/trackinformation such as client attendance, program expenses (i.e., buspasses and financial assistance given to participants). QualificationsEducation: Required: Bachelor's degree in human services or relatedfield, or equivalent work experience. Experience: Required: At least oneyear experience working with children. Preferred: 1+ years' casemanagement experience; substance use disorder and mental healthknowledge Skills and Competencies: Working knowledge of childdevelopment. Ability to establish and maintain effective workingrelationships. Ability to provide children with effective direction andsupervision. Ability to detect and recognize potentially hazardoussituations and take immediate and appropriate action. Willingness towork some evening hours required. Valid Wisconsin driver's license withreliable personal vehicle for transporting persons served. Maintainrequired auto insurance and acceptable driving record requirements. WhyJoin Us?
