Full-time (80 hours biweekly) Weekdays, daytime hours between 8 am - 7 pm Sign On Bonus Eligible Program Summary Pathways to Hope is a specialty care program coordinated for individuals ages 15-30 experiencing their first symptoms of psychosis. A team of providers, including a psychiatrist, licensed therapist, case manager, and peer support specialist provide evidence based, recovery oriented interventions designed to build resilience, reduce the impact of symptoms on functioning and support individuals in pursuing their goals. Group and individual services are provided in office and in the community. Treatment is individualized and is a collaborative process between the individual and the treatment team. General Summary Provides individual, group and/or family therapy. Provides preadmission screening to determine appropriateness for admission. Interviews patients and/or families to obtain psycho-social histories. Collaborates with the treatment team to develop a treatment plan and coordinates discharge planning. Duties and Responsibilities Essential Functions: + Conducts individual or group therapy sessions. Assesses patients' progress and modifies or extends treatment program accordingly. + Provides therapeutic intervention with patient and family including individual, marital, parental, family and group therapies. + Interviews patients and family members to obtain information about home environment, family relationships, health history, and patient's personality traits. + Documents current, accurate observations of care to the patient in appropriate records. + Coordinates care of assigned patients. + Assists in the implementation of education and training experience for interns and other designated students. + Prepares and maintains written documentation as required by the profession and the department. Common Expectations: + Demonstrates knowledge of the principles of growth and development over the life span of the assigned patient population. Assesses and interprets patient age specific data and provides appropriate, age specific treatment. Provides direct patient care to assigned patient age group(s). + Demonstrates a commitment to patient, visitor and staff by: complying with all applicable safety regulations; learning the impact of medical errors and methodology that will lead to reduction of errors; reporting actual and potential errors, as well as hazardous conditions; identifying opportunities to standardize processes and
