Responsibilities include direct patient care for general medicine, hospitalist medicine and surgery inpatient services, including daily patient care rounds, patient education, and facilitating transitions of care. Furthermore, the clinical pharmacist will also be an active contributor to the internal/hospitalist medicine area by working to develop policies, procedures, guidelines and protocols. Participation in the training and education of UChicago’s pharmacy residency program, Chicagoland pharmacy students, as well as the Graduate Medical Education residents and physicians is expected. Research and quality improvement are high priority activities for the clinical pharmacy team. In this role, the Clinical Pharmacist Specialist will collaborate with pharmacy and interdisciplinary colleagues to accomplish patient-centered goals.Eligible for a Sign-On Bonus up to $5,000Job SummaryThrough various assignments within the department, the Clinical Pharmacy Specialist provides support of centralized and decentralized medication-use systems and delivers optimal medication therapy to patients with a broad range of disease states. Clinical Pharmacist Specialists are able to participate in all necessary aspects of the medication-use system while providing comprehensive and individualized pharmaceutical care to the patients in their assigned areas. Pharmaceutical care services include but are not limited to assessing patient needs, incorporating age and disease specific characteristics into drug therapy and patient education, adjusting care according to patient response, and providing clinical interventions to detect, mitigate, and prevent medication adverse events. Clinical Pharmacist Specialists serve as departmental resources and liaisons to other departments, hospital personnel, or external groups. They proficiently manage and facilitate the appropriate use of medications according to professional and regulatory standards. They also conduct clinical research and practice advancement projects as well as patient care quality and regulatory compliance initiatives designed to improve medication-use processes or pharmacy practice. Clinical Pharmacist Specialists provide medication and practice-related education/training and actively serve as preceptors for doctor of pharmacy students and pharmacy residents.Essential Job FunctionsLeads assigned patient service lines, clinical areas, and therapeutic programsDelivers direct patient care and clinical practice, including decentralized and service-based programsMaintains a standard of performance that is in compliance with Joint Commission, ASHP practice standards/guidelines, federal/state licensing acts, and UCM and Pharmaceutical Services Policies and ProceduresFacilitates experiential education and practice advancementPerforms other duties as assigned (including but not limited too TDM, PK and AC monitoring, and cardiac arrest response) Required QualificationsGraduate of an ACPE accredited College of PharmacyPGY1 residencyPGY2 Internal Medicine Residency, or equivalent of 3-5 years experience with the patient populationKnowledge of contemporary hospital/clinical practice and serviceKnowledge and understanding of general hospital operations and regulatory complianceAbility to integrate clinical and distributive pharmacy services with quality improvement, teaching, research, and professional development activitiesSignificant general clinical knowledge and expertise in traditional therapeutics and patient centered careEligible for licensure as a pharmacist in the state of Illinois or eligible (complete licensure process within 60 days of employment)Board Certification in area of expertise is required within 18 months of hire (ie.
