Bonus: $5,000 Location:u00a01900 Ballington Blvd NW, Rochester, MN 55901 Physical Therapist Responsibilities: + Assess & Diagnose:u00a0Evaluate patients' physical conditions, movement abilities, and medical history to develop personalized treatment plans. + Develop & Implement Treatment Plans:u00a0Create and execute rehabilitation programs to improve the functional deficits of the patient. + Provide Hands-on Therapy:u00a0Perform manual therapy, therapeutic exercises, and use modalities such as but not limited to ultrasound and electrical stimulation as needed. + Educate & Support Patients:u00a0Provide patient, caregiver, staff education as deemed clinically necessary such as but not limited to: pain management techniques and fall risk safety. Teach patients and families about exercises, injury prevention, and pain management techniques. + Monitor Progress & Adjust Plans:u00a0Track patient outcomes, document progress, and modify treatment plans as needed. + Collaborate with Healthcare Teams:u00a0Work with doctors, nurses, and other therapists to ensure comprehensive patient care. Physical Therapist Qualifications: + Must be a graduate from an Accredited Physical Therapy Program and have a bacheloru2019s degree + Must possess a current accredited MN Physical Therapist License + Must possess strong knowledge regarding state, federal, and local regulations as they pertain to Home Health care. + Must be capable of performing the essential functions of the job, with or without reasonable accommodations. + Must be able to communicate in English, both verbally and in writing and possess sufficient communication skills to perform the tasks required.
