Virtual Hospitalist, Advanced Care at Home u2013 Parkline - Chapel Hill, NC Command Center u2013 Parkline - is considered u201cplace of businessu201d for this role located in Chapel Hill, NC The Virtual Hospitalist of UNC Healthu2019s Advanced Care at Home program is a dynamic, experienced provider who is passionate about transforming the future of healthcare. Under the program, UNC Health physicians and nurses will monitor and communicate with patients in their homes 24/7 from a clinical command center located in Chapel Hill, NC. The virtual team will deploy a network of rapid response home based services to patients as needed. Patients will receive in person assessments by nursing, paramedics and NPs, as well as virtual assessments by nurses and physicians. This role provides the following opportunities: u2022 Work in a team environment to provide high-quality inpatient care to patients in their own homes through Advanced Care at Home (ACH). u2022 Clinical outcomes are equal to or better than inpatient hospitalization with improved patient experience u2022 State-of-the-art equipment at Command Center and patientsu2019 home u2022 Patients have access to hospital services and more including medication management, mobile imaging, home infusion, meal delivery, nutrition consults, phlebotomy, & rehab services u2022 Integrated care with primary care providers, specialists & ancillary departments Responsibilities u2022 Lead a team of Advanced Practice Providers and nurses to coordinate care for patients in ACH u2022 Perform the initial virtual history and physical for new patient admissions after patient arrives home u2022 Provide daily virtual visits from the clinical command center in Chapel Hill (Must be onsite for a few days a week) u2022 Collaborate with the Advanced Practice Providers to develop and implement a plan of care, including diagnostic and specialty consultations virtually or as day trips to brick and mortar facilities u2022 Provide safe handoffs to the colleagues and Command Center Registered Nurse, as well as patientu2019s primary care physician during hospitalization as needed and at discharge u2022 Perform clinical screenings on eligible patients at eligible brick and mortar facilities u2022 Communicate with the patient and family concerning the ACH model, incorporating patient and family values and goals in the plan of care u2022 Ensure census growth by identifying appropriate patients for the ACH model which includes patient chart reviews, outreach to brick-and-mortar provider teams, and coordination with internal acquisition team u2022 Keep abreast and be willing to maintain flexibility as program changes and evolves u2022 Practice in accord with practitioner guidelines per state and hospital regulations u2022 Command Center u2013 Parkline - is considered u201cplace of businessu201d for this role located in Chapel Hill, NC.
