Summary The scope of practice of the Clinical Pharmacist Specialist covers direct and indirect patient care, education, medication safety, performance improvement, and consultation. The Clinical Pharmacist Specialist is responsible for the provision of both operational and clinical pharmacy services. This position provides direct clinical pharmacy services to inpatient malignant hematology, medical oncology, and gynecologic oncology patient populations. The hematology/oncology pharmacy service emphasizes the identification, resolution, and prevention of medication-related issues within these specialized populations. The Clinical Pharmacy Specialist is responsible for reviewing, coordinating, and overseeing all inpatient chemotherapy needs, including verification and monitoring of chemotherapy protocols and desensitization regimens, responding to regimen-based clinical inquiries, and providing medication-related education to patients, caregivers, and members of the interdisciplinary healthcare team. Responsibilities + u00b7 Responsibilities will include order verification, discharge counseling, projects, and education of other healthcare professionals. + u00b7 Evaluate and verify Epic physician's orders for therapeutic appropriateness, proper dosing, optimal dosage form, drug interactions, allergic reactions, route, and rate of administration and therapeutic redundancy. + u00b7 Clarify physicians' orders when necessary through direct and timely contact with prescriber or designated practitioner. + u00b7 Provides patient-specific pharmacotherapy recommendations and drug information to medical and nursing staff. + u00b7 Participates in the pharmacy department's most current method of documenting clinical interventions. + u00b7 Perform daily review of pharmacy reports including pharmacokinetic monitoring and other drug or patient specific reports that may be required. + u00b7 Conducts monthly nursing unit inspections of medication storage areas as assigned. + u00b7 Supervise pharmacy technician staff involved in the preparation and delivery of medications ensuring coordination of workflows with established delivery schedules. + u00b7 Coordinate medication ordering, preparation, dispensing, and administration systems for the patient populations outlined above. + u00b7 Implements all drug use policy and formulary decisions of the Pharmacy and Therapeutics committee. + u00b7 Participates in Safety Event (ADR and medication error) reporting programs. + u00b7 Complete clinical and operational activities as designated by the departmental management team (e.g. pyxis inventory optimization, relevant process improvement projects, etc.). + u00b7 Precept pharmacy students and residents (PGY1 and PGY2) as assigned in clinical and operational areas. + u00b7 Assess and evaluates the medical and drug literature in order to compare and contrast therapeutic regimens and uses for new or existing drug treatments. + u00b7 Implement medication safety initiatives as determined by current departmental needs and ongoing projects. + u00b7 Develop operational and clinical projects as determined by current departmental needs and ongoing projects. + u00b7 Serves as a liaison with other departments for collaborative research activity. + u00b7 Responsibility for evaluating and monitoring drug therapy relative to the total needs/problems of the ambulatory care patient in collaboration with physicians, nurses and other healthcare professionals. + u00b7 Reconciles home medications for admitted patients. + u00b7 Attends interdisciplinary rounds for malignant hematology population to assist with the design, implementation and monitoring of medication regimens. + u00b7 Reviews chemotherapy orders for hematology, oncology, gyne/onc patients receiving chemotherapy inpatient. + u00b7 Assist with facilitating inpatient chemotherapy desensitization regimens. Other Information Required Job Qualifications: + u2022Bachelor of Science in Pharmacy. + u2022Illinois Pharmacist License. + u2022Minimum of four years of Clinical Pharmacy experience OR PGY1 residency completed within the last seven years, which includes Clinical / Specialty Pharmacy experience. + u2022Knowledge in intravenous admixture services, intravenous drug therapy and chemotherapy. + u2022Must be able to work effectively with others to provide safe and effective drug therapy. + u2022Excellent problem solving skills and the ability to work independently and quickly in responding to and resolving a wide range of problems. Preferred Job Qualifications: + u2022BLS Certification. + u2022ACLS Certification (or shall be obtained upon hire). + u2022Board of Pharmaceutical Specialties Board Certification. + u2022Doctor of Pharmacy degree and residency certificate. + u2022Experience and strong knowledge base in hematology/oncology.
